How the Professional Referral Pathway Works
1. Submit Professional Contact Information
Provide your facility or organization, role, direct contact details, and general service interest—without PHI.
2. Direct Follow-Up
A member of Loving Private Care 71 follows up with the referring professional to discuss the general need and secure next steps.
3. Client or Family Coordination
When authorized and appropriate, the care conversation continues with the client, family, or designated representative.
